All teams are guaranteed three (3) futsal matches leading into single-elimination playoffs. Matches will consist of two 12-minute halves with a running clock and on-the fly substitutions. Teams will have a two (2) minute half-time. Roster sizes will be limited to 12 players. Players may only be rostered to one team per gendered age group. Players may play up at their team's discretion. Team entry fee $360 plus $10 cash referee fee per match, per team paid before the start of the match. 

All matches will be held on high-quality, FIFA regulation futsal courts. Match rules will be to modified FIFA Futsal Laws of the Game. 

$360 team entry fee  – Final payments will not be due until your team has been notified of tournament acceptance. Your card will not be charged. This is a pre-registration to reserve your spot until your payment is processed in the future. By completing this page, you agree that this payment information will be used to process your payment for the full amount at the proper time. Cards used for pre-registration will see a $1 pre-authorization charge that is immediately reversed in order to verify the card is valid. Clubs entering four (4) or more teams on one registration submission will have an automatic discount applied.

Check payments will be available and should be made payable to Matrix Soccer Academy. Teams who have not paid by team check-in will be marked as a tournament forfeit. Team check-in should be no later than one hour before team's first match. Team check in requirements will include final rosters, proof of age* for each rostered player, and completed Crown Sports Center waiver for each rostered player. Team check-in documents will be provided to email addresses on file for submission upon tournament acceptance. Team check-in documents may be turned in ahead of tournament to [email protected] but no later than one hour before team's first scheduled match.

*Acceptable proof of age includes: player birth certificate, player passport, USYS, US Club Soccer, US Futsal, or USYF player cards

A signed waiver by event participant or parent/guardian will be required before you can participate in any activities hosted at Crown Sports Center. This additional waiver will be provided in team check-in packets and must be completed by all rostered players to be submitted no later than team check-in.
In consideration of being allowed to participate in the event or activity referenced above, I acknowledge, appreciate, and agree that: 1) The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and, 2) I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and, 3) I willingly agree to comply with the stated and customary terms and conditions for participation. In addition, I hereby grant the right of possession of photographs taken during the course of the event by event organizers and have express permission to make use of said images for their personal or commercial use in any forum.
